Insurance companies require anyone planning to have the bariatric weight-loss procedure to have a psychological evaluation. This evaluation assesses an individual’s appropriateness for the Bariatric Surgery and post-operative compliance with dietary requirements and procedures. The evaluation includes a clinical interview as well as the completion of a diagnostic questionnaire, and other forms that will be provided to you when you come to your appointment. Then, a psychological report will be written and faxed to your surgeon. You must sign a “release of information” form in order for your surgeon to receive the fax. This release form will be provided to you when you come to your appointment.
